Mail Form W-7, your tax return (or other documents required by an exception), and the documentation described in item (3) under How to Apply and listed in the chart above to:

You can apply for an ITIN by bringing your completed forms and documentation to any IRS Tax-payer Assistance Center in the United States and most IRS offices abroad, where IRS personnel will verify your documentation and forward the application to the IRS processing center.
